We’re excited to welcome new patients to VK Dental Care! Our friendly team is here to help you achieve the healthy, beautiful smile you’ve always wanted. If you have any questions about your treatment options, don’t hesitate to ask during your visit.
Before your first appointment, please follow the link NEW PATIENT FORM to complete your paperwork from your phone, tablet, or computer. At the end of the form, you’ll have the option to upload your insurance card. If you’re unable to fill out the forms online, please arrive a bit early and bring your insurance card with you if possible. If you’re new to the area, you may also request your previous dentist to send us your dental records.
If you need antibiotics before dental work, take them as prescribed. If you’re unsure, please check with your doctor before your visit.

Returning Patients:
Welcome back! As part of our commitment to going paperless, we ask that all returning patients complete a quick update form before each visit. Please follow the link RETURNING PATIENT FORM to fill out the returning patient form from your phone, tablet, or computer. This ensures we have your most current information and can continue providing the best care possible.
Visiting Policy:
To maintain a comfortable and safe environment for all our patients, we kindly ask that no more than one relative, guardian, or friend accompany you to your appointment. This helps us ensure that our office remains a calm and efficient space for everyone.
Insurance and Payment Options:
Dental insurance varies significantly depending on whether it is provided by the government or through your employer. Employer-sponsored dental benefits are based on the specific agreement between your employer and the insurance carrier. On the other hand, government insurance may have different coverage levels and limitations.
We’ll provide an estimate of your coverage at the time of treatment, but please note that this is not a guarantee of payment. We’ll handle the insurance claims for you, but any remaining balance is your responsibility. If the insurance payment differs from our estimate, we’ll adjust your bill accordingly.
We accept all major credit cards for your convenience.
